HOSPITAL DISCHARGES & HOME SAFETY ASSESSMENT
Perhaps you or a loved one has had a recent hospital admission or acute injury, or maybe things have been changing over time and you are wanting to plan ahead for your safety at home. This assessment focuses on creating a safe and supportive environment for daily living and smooth transitions home after hospitalization. It identifies risks and barriers in the home, considers the client’s physical and cognitive abilities, and ensures the right equipment and strategies are in place.
Key areas include:
Environment: Pathways, surfaces, stairs, lighting, and access to essential spaces.
Assistive devices & equipment: Mobility aids, grab bars, transfer supports, beds, rails, and lifts to promote safe transfers and independence.
Client abilities: Physical and cognitive factors that influence safe participation in daily activities.
Personalized recommendations may include home modifications, adaptive equipment, and caregiver training to maximize safety and independence. Support can also be provided for funding applications requiring occupational therapist input.
