HOSPITAL DISCHARGES & HOME SAFETY ASSESSMENT

Perhaps you or a loved one has had a recent hospital admission or acute injury, or maybe things have been changing over time and you are wanting to plan ahead for your safety at home. This assessment focuses on creating a safe and supportive environment for daily living and smooth transitions home after hospitalization. It identifies risks and barriers in the home, considers the client’s physical and cognitive abilities, and ensures the right equipment and strategies are in place.

Key areas include:

  • Environment: Pathways, surfaces, stairs, lighting, and access to essential spaces.

  • Assistive devices & equipment: Mobility aids, grab bars, transfer supports, beds, rails, and lifts to promote safe transfers and independence.

  • Client abilities: Physical and cognitive factors that influence safe participation in daily activities.

Personalized recommendations may include home modifications, adaptive equipment, and caregiver training to maximize safety and independence. Support can also be provided for funding applications requiring occupational therapist input.

MOBILITY ASSESSMENT

The mobility assessment evaluates a client’s ability to move safely and effectively within their environment. This includes assessing transfers, ambulation, wheelchair use, posture and positioning, and the safe use of mobility aids (e.g., manual wheelchairs, power wheelchairs, walkers, scooters). The assessment considers physical, cognitive, and environmental factors that may impact safe mobility and functional independence.

Findings from the mobility assessment guide the development of individualized interventions, environmental modifications, equipment recommendations, and safety strategies to support independence, participation, and fall prevention.

Nicole is a registered occupational therapist with the Assistive Devices Program of Ontario (see eligibility criteria here ) and can assist with funding applications for basic and essential mobility equipment. She has extensive experience prescribing both standard and complex custom wheelchair and seating systems.

CAREGIVER/STAFF EDUCATION

We provide education for staff and caregivers on safe client handling to reduce injury risk and promote client independence.

Training covers:

  • Client Assessment: Understanding physical and cognitive abilities to plan safe transfers.

  • Body Mechanics: Using proper posture and techniques to prevent caregiver injury.

  • Assistive Devices: Safe use of wheelchairs, gait belts, slide sheets, and mechanical lifts.

  • Transfers & Repositioning: Bed-to-chair, sit-to-stand, and in-bed movements.

  • Environmental Safety: Clear pathways, secure furniture, and hazard-free spaces.

  • Communication: Engaging clients and coordinating with caregivers during tasks.

Our goal is safe, dignified mobility for clients while protecting caregivers and promoting independence.

Nicole French is a compassionate occupational therapist based in Hamilton, Ontario, dedicated to helping clients achieve independence and meaningful participation in daily life. She holds a degree in Occupational Therapy from McMaster University and uses a client-centered, evidence-based approach tailored to each individual’s goals and abilities.

Her experience spans inpatient and outpatient neurological rehabilitation, complex wheelchair and seating assessments, and community care, providing therapy in homes and accessible community settings. Nicole specializes in mobility, adaptive equipment, home safety, and functional strategies to support everyday living.

Passionate about outdoor and nature-based interventions, she strives to make activities safe, accessible, and engaging for all clients. Nicole works closely with clients, families, and multidisciplinary teams to create personalized plans that enhance safety, independence, and overall well-being.
